| To participate in the athletic program, a student must have had a sports physical current within a year during the entire season they participate in. It cannot expire anytime from the first date of practice to the last game. It is recommended that a student have the physical during the summer especially for multiple sport athletes. The only other "safe time" is within a few days of either November 15 or March 15. This physical must be on record in the school nurse's office. An Emergency & Permission form must be on file before a student participates in a sport. This form will be given to students before a sport starts. The Emergency and Permission form can be also downloaded from the link below. You will need the free Acrobat Reader.
